On Instagram, "mutuals" are accounts that follow each other — you follow them and they follow you back. The word is also used more loosely for people you interact with regularly even without a formal follow-back. Mutuals usually make up the core of someone's real social circle, which is why an unfollow between mutuals tends to feel more significant than losing a random follower.
Checking mutuals is common when people want to understand a social graph: who two accounts have in common, or whether a follow is reciprocated. Instagram shows "Followed by [name] + others" on a profile, which surfaces some — but not all — mutual connections.
A one-sided follow (you follow them, they don't follow back) is the opposite of a mutual, and is what "who doesn't follow me back" tools are built to surface.
